This includes providing extra help to conduct clinical medicines reviews, manage long-term conditions like hypertension, delivering advice for those on multiple medicines (polypharmacy), medicines safety audits & education and better access to health checks. They also assist with communication across a patient’s care pathway, manage medicines shortages by suggesting suitable alternatives where appropriate, and mentor newer pharmacists.
